{\centering\bfseries Supplemental documentation for hpstat \textit{intcox} command\par}
The hpstat \textit{intcox} command implements the Cox proportional hazard model for interval-censored data, using an expectation–maximization-type algorithm by Zeng, Mao \& Lin [1]. Some technical details omitted by Zeng, Mao \& Lin, and some simplifications made possible by \textit{intcox} supporting only time-independent covariates and a proportional hazards model, are described here.
The hpstat \textit{intcox} command implements Cox proportional hazards regression for interval-censored observations, using an iterative convex minorant algorithm. The general algorithm is proposed by Huang \& Wellner [1]. When computing the baseline hazard, we apply a damped iterative convex minorant algorithm described by Aragón \& Eberly [2] and Pan [3]. This documentation discusses technical details of the implementation.
Zeng, Mao \& Lin describe the model in terms of a transformation $G(x) = -\log \int_0^\exp(-xt) f(t) \ \mathrm{d}t$, where $f(t)$ is a density function. When $f(t)$ is the gamma density with unit mean and variance $r$, $G(x) = r^{-1} \log(1 + rx)$. When $r = 0$, $G(x) = x$ and this corresponds with the proportional hazards model, which is the only transformation supported by \textit{intcox}.
Let the baseline cumulative hazard be a step function $Λ(t)$ with steps at times $\{t_1, t_2, …, t_m\}$. We seek to optimise for the vector $\symbf{Λ} = (Λ(t_1), Λ(t_2), …, Λ(t_m))^\mathrm{T}$. The cumulative hazard $Λ(t; \symbf{Z}_i)$ given covariates $\symbf{Z}_i$ is $\exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) Λ(t)$. The cumulative distribution function for failure times is $F(t; \symbf{Z}_i) = 1 - \exp\left(-\exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) Λ(t)\right)$.
In the E-step, the posterior means for latent variables $\hat{E}(W_{ik})$ and $\hat{E}(ξ_i)$ are calculated.
Take the $i$-th observation, whose failure time falls in $(L_i, R_i]$.
For $\hat{E}(W_{ik})$, when $t_k ≤ L_i$, $\hat{E}(W_{ik}) = 0$, and when $L_i < t_k ≤ R_i$ for $R_i < ∞$:
%
$$\hat{E}(W_{ik}) = λ_k \exp(\symbf{β}^\mathrm{T} \symbf{Z}_{ik}) × \frac{\int_{ξ_i} ξ_i \{\exp(-ξ_i S_{i1}) - \exp(-ξ_i S_{i2})\} [1 - \exp\{-ξ_i (S_{i2} - S_{i1})\}]^{-1} f(ξ_i)\ \mathrm{d}ξ_i}{\exp\{-G(S_{i1})\} - \exp\{-G(S_{i2})\} }$$
%
Since, in the proportional hazards model, $G(x) = x$, and $f(\cdot)$ has gamma density with unit mean and 0 variance, $ξ_i = 1$ unconditionally. And since all covariates are time-independent:
The log-likelihood $\mathcal{L}_i$ for the $i$-th observation is:
%
\begin{align*}
\hat{E}(W_{ik}) &= λ_k \exp(\symbf{β}^\mathrm{T} \symbf{Z}_i) × \frac{\{\exp(-S_{i1}) - \exp(-S_{i2})\} [1 - \exp\{-(S_{i2} - S_{i1})\}]^{-1}}{\exp(-S_{i1}) - \exp(-S_{i2}) } \\
&= \frac{λ_k \exp(\symbf{β}^\mathrm{T} \symbf{Z}_i)}{1 - \exp(S_{i1} - S_{i2})}
\mathcal{L}_i &= \log( F(R_i; \symbf{Z}_i) - F(L_i; \symbf{Z}_i) ) \\
&= \log\left( \exp\left(-\exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) Λ(L_i)\right) - \exp\left(-\exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) Λ(R_i)\right) \right) \\
\intertext{Let $S_{Li} = \exp\left(-\exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) Λ(L_i)\right)$, and $S_{Ri} = \exp\left(-\exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) Λ(R_i)\right)$, so:}
\mathcal{L}_i &= \log\left( S_{Li} - S_{Ri} \right)
\end{align*}
%
In the M-step, Zeng, Mao \& Lin require the parameters $\symbf{β}$ to be updated by solving the following equation, whose left-hand side we denote $\symbf{h}(\symbf{β})$, using the one-step Newton–Raphson method:
Note the gradient $\nabla_{\symbf{Λ}} S_{Li}$ with respect to $\symbf{Λ}$ is the vector whose $j$-th element is:
%
$$\symbf{h}(\symbf{β}) = \sum_{i=1}^n \sum_{k=1}^m I(t_k ≤ R^*_i) \hat{E}(W_{ik}) \left\{ \symbf{Z}_{ik} - \frac{\sum_{j=1}^n I(t_k ≤ R^*_j) \hat{E}(ξ_j) \exp(\symbf{β}^\mathrm{T} \symbf{Z}_{jk}) \symbf{Z}_{jk}}{\sum_{j=1}^n I(t_k ≤ R^*_j) \hat{E}(ξ_j) \exp(\symbf{β}^\mathrm{T} \symbf{Z}_{jk})} \right\} = \mathbf{0}$$
$$\frac{\partial S_{Li}}{\partial Λ(t_j)} = - S_{Li} \exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) \mathrm{I}(t_j = R_i)$$
%
Since $\hat{E}(ξ_i) = 0$, and all covariates are time-independent:
$\nabla_{\symbf{Λ}} S_{Ri}$ is derived similarly, and so the gradient $\nabla_{\symbf{Λ}} \mathcal{L}_i$ is the vector whose $j$-th element is:
%
$$\symbf{h}(\symbf{β}) = \sum_{i=1}^n \sum_{k=1}^m I(t_k ≤ R^*_i) \hat{E}(W_{ik}) \left\{ \symbf{Z}_i - \frac{\sum_{j=1}^n I(t_k ≤ R^*_j) \exp(\symbf{β}^\mathrm{T} \symbf{Z}_j) \symbf{Z}_j}{\sum_{j=1}^n I(t_k ≤ R^*_j) \exp(\symbf{β}^\mathrm{T} \symbf{Z}_j)} \right\} = \mathbf{0}$$
$$\frac{\partial}{\partial Λ(t_j)} \log\left( S_{Li} - S_{Ri} \right) = \frac{A_{ij}}{S_{Li} - S_{Ri}}$$
%
For each $k$, let $s_0 = \sum_{j=1}^n I(t_k ≤ R^*_j) \exp(\symbf{β}^\mathrm{T} \symbf{Z}_j)$, and $\symbf{s}_1 = \sum_{j=1}^n I(t_k ≤ R^*_j) \exp(\symbf{β}^\mathrm{T} \symbf{Z}_j) \symbf{Z}_j$, such that we now require:
%
$$\symbf{h}(\symbf{β}) = \sum_{i=1}^n \sum_{k=1}^m I(t_k ≤ R^*_i) \hat{E}(W_{ik}) \left\{ \symbf{Z}_i - \frac{\symbf{s}_1}{s_0} \right\} = \mathbf{0}$$
%
Denote by $\symbf{S}_2$ the Jacobian of $\symbf{s}_1$, with respect to $\symbf{β}$. This is the square matrix whose $(a, b)$-th element is:
%
$$\frac{\partial}{\partial β_b} \sum_{j=1}^n I(t_k ≤ R^*_j) \exp(\symbf{β}^\mathrm{T} \symbf{Z}_j) Z_{ja} = \sum_{j=1}^n I(t_k ≤ R^*_j) \exp(\symbf{β}^\mathrm{T} \symbf{Z}_j) Z_{ja} Z_{jb}$$
%
In other words, the Jacobian of $\symbf{s}_1$ is:
%
$$\symbf{S}_2 = \sum_{j=1}^n I(t_k ≤ R^*_j) \exp(\symbf{β}^\mathrm{T} \symbf{Z}_j) \symbf{Z}_j \symbf{Z}_j^\mathrm{T}$$
%
Notice also that $\symbf{s}_1$ is the gradient of $s_0$ with respect to $\symbf{β}$. Applying the quotient rule to $\symbf{s}_1 / s_0$, the Jacobian of $\symbf{h}(\symbf{β})$, then, is:
%
\begin{align*}
\mathbf{J}[\symbf{h}(\symbf{β})] &= \sum_{i=1}^n \sum_{k=1}^m I(t_k ≤ R^*_i) \hat{E}(W_{ik}) \left\{ -\frac{\symbf{S}_2 s_0 - \symbf{s}_1 \symbf{s}_1^\mathrm{T}}{(s_0)^2} \right\} \\
&= \sum_{i=1}^n \sum_{k=1}^m I(t_k ≤ R^*_i) \hat{E}(W_{ik}) \left\{ \frac{\symbf{s}_1}{s_0} × \frac{\symbf{s}_1^\mathrm{T}}{s_0} - \frac{\symbf{S}_2}{s_0} \right\}
\end{align*}
%
We then apply the Newton–Raphson method to $\symbf{h}(\symbf{β})$ as required by letting:
%
$$\symbf{β} := \symbf{β} - \left\{\mathbf{J}[\symbf{h}(\symbf{β})]\right\}^{-1} \symbf{h}(\symbf{β})$$
Where $A_{ij} = \partial\left( S_{Li} - S_{Ri} \right) / \partial Λ(t_j) = S_{Ri} \exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) \mathrm{I}(t_j = R_i) - S_{Li} \exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) \mathrm{I}(t_j = L_i)$. The sum of all $\nabla_{\symbf{Λ}} \mathcal{L}_i$ yields $\nabla_{\symbf{Λ}} \mathcal{L}$.
{\centering\scshape References\par}
Note that $\partial A_{ij} / \partial Λ(t_j) = -A_{ij} \exp(\symbf{Z}_i^\mathrm{T} \symbf{β})$, so applying the quotient rule and simplifying, the Hessian $\nabla^2_{\symbf{Λ}} \mathcal{L}_i$ has diagonal $(j, j)$-th elements:
%
$$\frac{\partial^2}{\partial Λ(t_j){}^2} \log\left( S_{Li} - S_{Ri} \right) = \frac{-A_{ij} \exp(\symbf{Z}_i^\mathrm{T} \symbf{β})}{S_{Li} - S_{Ri}} - \left( \frac{A_{ij}}{S_{Li} - S_{Ri}} \right)^2$$
%
And the sum of all $\nabla^2_{\symbf{Λ}} \mathcal{L}_i$ yields $\nabla^2_{\symbf{Λ}} \mathcal{L}$.
Let $G$ be a diagonal matrix of the diagonal elements of $-\nabla^2_{\symbf{Λ}} \mathcal{L}$. As discussed by Pan [3], we update $\symbf{Λ}$ by iteratively applying:
%
$$\symbf{Λ} \leftarrow \mathrm{Proj}[ \symbf{Λ} + α G^{-1} \nabla_{\symbf{Λ}} \mathcal{L}, G, \mathbb{R} ]$$
%
Where $\mathrm{Proj}$, as defined by Pan [3], essentially represents monotonic (isotonic) regression. We port an efficient pool adjacent violators algorithm (PAVA) implementation from scikit-learn.
Turning to the estimation of $\symbf{β}$, note the gradient $\nabla_{\symbf{β}} S_{Li}$ with respect to $\symbf{β}$ is:
%
$$\nabla_{\symbf{β}} S_{Li} = \frac{\partial S_{Li}}{\partial\symbf{β}} = -S_{Li} \exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) Λ(L_i) \symbf{Z}_i$$
%
$\nabla_{\symbf{β}} S_{Ri}$ is derived similarly. The gradient $\nabla_{\symbf{β}} \mathcal{L}_i$ is:
%
$$\nabla_{\symbf{β}} \mathcal{L}_i = \left[ \frac{B_{Ri} - B_{Li}}{S_{Li} - S_{Ri}} \right] \symbf{Z}_i$$
%
Where $B_{Li} = S_{Li} \exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) Λ(L_i)$ and $B_{Ri} = S_{Ri} \exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) Λ(R_i)$.
Note $\partial B_{Li} / \partial \symbf{β} = \exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) Λ(L_i) (S_{Li} - B_{Li}) \symbf{Z}_i$, and $\partial B_{Ri} / \partial \symbf{β}$ is analogous. Applying the quotient rule and simplifying, the Hessian $\nabla^2_{\symbf{Λ}} \mathcal{L}_i$ is:
%
$$\nabla^2_{\symbf{β}} \mathcal{L}_i = \left[ \frac{\exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) Λ(R_i) (S_{Ri} - B_{Ri}) - \exp(\symbf{Z}_i^\mathrm{T} \symbf{β}) Λ(L_i) (S_{Li} - B_{Li})}{S_{Li} - S_{Ri}} - \left( \frac{B_{Ri} - B_{Li}}{S_{Li} - S_{Ri}} \right)^2 \right] \symbf{Z}_i \symbf{Z}_i^\mathrm{T}$$
%
And the sum of all $\nabla^2_{\symbf{β}} \mathcal{L}_i$ yields $\nabla^2_{\symbf{β}} \mathcal{L}$.
As discussed by Huang \& Wellner [1], we now apply standard Newton–Raphson optimisation by iteratively applying:
%
$$\symbf{β} \leftarrow \symbf{β} + [-\nabla^2_{\symbf{β}} \mathcal{L}]^{-1} \nabla_{\symbf{β}} \mathcal{L}$$
%
We alternate between updating $\symbf{Λ}$ and updating $\symbf{β}$ until convergence is reached.
The covariance matrix for $\symbf{β}$ is computed as suggested by Zeng, Gao \& Lin [4] as the inverse of the empirical Fisher information, which is in turn estimated as the outer product of the gradient of the profile log-likelihood.
